Where it fits
Collects premiums from individuals and employers, negotiates package rates with hospital networks and pays claims - cashless through TPAs or by reimbursement. Government schemes (PM-JAY, CGHS, ECHS, state schemes) play the same paying role for their beneficiaries.
Why it matters
Insurance and government schemes now pay most private inpatient bills, so payers decide hospital pricing. Retail health premiums became GST-free from 22 Sep 2025 and health grew to about 40% of non-life premiums in FY26 - a deeper paying pool for hospitals, but also stronger buyers that push back on rates.
Where the value pool sits
Health premiums were ~₹1.37L Cr in FY26, up ~15% (General Insurance Council). Retail health is profitable for focused insurers (claims ratios in the 60s); group and government health often run near or above 100% combined ratios. TPAs earn a fee per policy or claim.
Key drivers
GST exemption on individual policies (Sep 2025)
IRDAI push for 'Insurance for All by 2047' and faster cashless claims
Rising awareness after COVID; employer cover expanding
Potential bottlenecks
Low penetration in the 'missing middle' outside schemes and employer cover
Medical inflation pushing claims ratios up
Disputes with hospital associations over package rates and cashless access
